George Monbiot (Guardian Nov 15th 2005) comments on the Italian TV documentary supposedly showing that the US used phosphorous in Iraq. He points out at least two newspaper reports by embedded reporters which clearly state that US forces used "shake-n-bake" phosphorous weapons.
